The Avenue offers exceptional Assisted Living.

 

The Avenue is licensed for ambulatory and non ambulatory residents by the state of California as an RCFE.

 

The Assisted Living and Health Services Programs are coordinated and supervised by a Licensed Nurse.

 

The Avenue can accommodate residents requiring personal care assistance which may include medication monitoring, grooming and bathing assistance, ‘round-the-clock well-being monitoring, personal laundry, and stand-by or hands-on assistance with one or more of the activities of daily living.

 

Basic services in assisted living include a 24-hour staffing and emergency response system, three meals per day, weekly housekeeping and linen service, planned activities and scheduled transportation.

Charges for individual personal care service plans are determined through an assessment process using a point-based scoring method. Points are determined by the frequency and duration of your individual service needs, as well as direct and indirect cost of the personnel performing the service. The assessment will reflect a point total and a total charge for the services provided.
